We offer THREE options for enrollment:
1. ONLINE ENROLLMENT / PAY ELECTRONICALLY
This option has an online enrollment form which allows you to enter all your information online, upload vaccination records, select your classes, choose your payment method (credit card, ApplePay, GooglePay, CashApp pay or bank transfer), and submit the form.

–> When you choose Submit on the online enrollment, you will receive a confirmation page and email. If nothing happens when you choose Submit, it means a required field is not completed. Scroll up and fill in all required fields as needed. Please be sure you receive a confirmation page and email after submitting your online form.
2. ONLINE ENROLLMENT / PAY BY PAYPAL OR CHECK
This option has an online enrollment form which allows you to enter all your information online, upload vaccination records, select your classes, submit your information, and then pay separately by PayPal or check.

3. PAPER ENROLLMENT BY MAIL / PAY BY CHECK
For each paper enrollment, please make sure you include the following when mailing your information:
1. Completed enrollment form
2. Updated vaccination records
3. Check payable to OTCB, Inc.
